Primary responsibilities include overall health care for all children in out of home care; coordination of health care services; systematic communication and coordination of care among medical personnel, caseworkers, and caregivers; advocate to ensure that children receive all health services in a timely manner; educating full case management agency staff, caregivers and health care professionals about health care and foster care issues; develop and maintain collaborative relationships with full case management providers, medical providers and community-based programs; participate in work groups and committees as assigned.
This position requires an RN License. Job Duties: 1. Verifying health care information, 2. Communicating with caregiver for children upon removal from home due to abuse or neglect, assisting in understanding medication administration, 3. Face to face consultation as needed, 4. Answering all health related questions until the child is linked to a medical home, 5. Health care information gathering, 6. Ensuring medical consents are available, 7. Coordination of health care services which includes: a. ensuring children have access to health care, b. monitoring the children’s use of the health care system, and c. ensuring children receive services appropriate to their health care needs, 8. Systematic communication and coordination of care among medical personnel, caseworkers, and caregivers, 9. Advocate to ensure that children receive all health services in a timely manner, 10. Educating full case management agency staff, caregivers and health care professionals about health care and foster care issues, 11. Develop and maintain collaborative relationships with full case management providers, 12. Medical providers and community-based programs; and 13. Participate in work groups and committees as assigned. Skills / Requirements
